What is your visa costs on a business application, and if this is the right form to fill?

The president of our disabled organization has to attend a seminar/workshop in Beijing from the 6/12 - 10/12/2011. He is on wheelchair and is inquiring if he could be assisted in getting some feed back soon on the question raised above.
