1)Did Malaysian citizen need to apply visa to Hong Kong? 2) Did Pakistan citizen holding pakistab passport need Visa and which form should be fill up?

Malaysian citizen don't need a visa to Hong Kong because they are exempt from a visa to Hong Kong for up to 90 days. Pakistan citizen need a visa to Hong Kong. He should fill Hong Kong Visa Application Form ID 1003A (Application for Entry for Visit/Transit in Hong Kong)
