1. get a single status certificate and make it notarized by a local Notary Public Office in India
2. make it attested by the CPV Division of the Ministry of External Affairs of India (Add: Patiala House, New Delhi), or the Branch Secretariat of the Ministry of External Affairs in Chennai (Add: EVK Sampath Maligai, 7th Floor, 68, College Road, Chennai-600 006).
3. make it attested by China embassy or consualte in India.

The Chinese Embassy or Consulates General in the Republic of India will only attest documents from their respective consular district.

Applicants from the state of Maharashtra and Karnataka may apply at the Consulate General of China in Mumbai.

And applicants from the state of West Bengal, Orisssa, Chhattisgarh, Jharkhand and Bihar may apply at the Consulate General of China in Kolkata.

Applicants from the rest of India may apply at the Chinese Embassy in New Delhi.

Point 1 says, "get single status certificate"? How do I do that? What is the format? Do I / my parents make an affidavit? Or does a 3rd-party make the certificate?

You should get from the marriage registration office in your hometown. When you go there, the marriage registration officer will issue a certificate to you. An affidavit may be needed.

Approach your VAO. You will have to give a application requesting the certificate. In few days they would enquire with your neighbors and VAO will issue a certificate in your local administrative language. With this certificate you will have to get verified by Revenue Inspector. After this you can take this to Tahsildar translated in English. He will seal and sign. After this you will have to get Apostille with the MEA at a Regional Attestation Center (RAC). Then after this attestation you can submit to Consular of that respective country for attestation. So simple ;-)
