where can I get all the paperwork to renew a chinese passport

my wife a chinese lady passport is about to expire where can i get the paperwork to do it before i go to chicargo

You need prepare following documents for passport renewal:
1. One application form for China passport, it can be downloaded from website of Chinese consulate in Chicago (in Chinese version);
2. Two recent passport-sized photos;
3. old passport and copies of its information page;
4. US green card or visa and copies.
Other documents if visa officers require. The processing takes 15 days in general.
