What are the requirement for student visa in China, both for undergraduate or postgraduate?

To get a China study visa, you are supposed to provide the following documents:
1. Original passport with enough blank visa pages and at least six months validity period left before expiration
2. A passport-style photo affixed to a properly signed application form
3. Your admission letter from your receiving school
4. JW202 form
5. Physical Examination Form for Foreigners issued from an authorized hospital
