About:Chinese Embassy in U.S.A.

I am planning on getting a "Z" work visa in the upcoming days and will shortly be mailed the needed paperwork from my employer in China. My supervisor told me that I can do the health check while in China, but I have read that I would need a filled out health check form in order to get the visa. Which one is true?
Also, besides a copy of my passport, passport picture, and proper letters, is there anything else I need to apply for the Visa? Is there a form I need to fill out?
What is the fastest I can get this visa processed?

You may fill the first part of the physical examination form for the application of the form and do the health check when you are in China.
Yes, you still need a completely filled visa application form affixed with visa-sized photo and Working Permits, such as: Working Permit issued to foreign experts by The State Administration of Foreign Experts Affairs, Alien Employment License of the People's Republic of China issued by Ministry of Labor and Social Security, Letter of Invitation for Foreigners Engaged in Offshore Petroleum Operations in the People's Republic of China, etc.
The fastest way is you go to the China embassy or consulate near to apply for the visa in person in early morning and use the same day rush service.
