About:Chinese Embassy in Japan

I am an American living in Japan. I want to visit China with my 14 year old daughter. I have three questions:
1) does my daughter need to go with me to apply for a visa or can I just take her passport and picture?
2)is there any other consulate closer to Aizu than tokyo where I can appy.
3)in July and August, other than July 20 are there any other Mondays the consulate will be closed?
