About:Chinese Embassy in Canada

Hello,

Can you please explain to me how I can apply to have an affidavit single status certificate form notarized? I hope to be able to get married in China. I could not find a standard "single status form" but I have copied and completed a form on the internet that an other gentleman has drafted for this very purpose. I understand the form has to be notarized by your embassy in Ottawa and I will also need copy in Chinese. If I take my completed form with my passport and supporting documentation to prove who I am and that I am single to your embassy can I have the form notarized and translated to Chinese?
