Q1 visa requirements , medical test?

From pakistan want to visit family reunion, applying Q1 VISA IN ISLAMABAD, NEEDED ANY MEDICAL TEST? NEEDED any medical test when extend q1 visa inside china and applying temporary residence?
what documents required to extend Q1 VISA INSIDE CHINA AND TEMPORARY RESIDENCE?
CAN Q1 visa be change into z visa inside or needed to back home country and apply again?

As I know, it generally doesn't need any medical test during the applying process. However, when applying for a temporary residence and visa extending, it requires a medical test.

It is nearly not possible to change your visa in China and you have to apply one in your own country.

Thanks for reply, so medical required inside china,so no needed any medical test from Pakistan to apply temporary residence on behalf Q1 visa, this is what I understood.Hace any idea how much it cost inside china?
