About:Chinese Consulate in Chicago

I have work permit issued in January 2021, want to go back on 6/4/2021

What documents do I need and where to submit? I have taken chinese vaccine before coming to USA

You need to submit the work permit together with your passport, recent photo, and fully completed China visa application form to local China Visa Application Service Center to get a Z visa, holding which you can enter China.
