I have been a housewife since my children were born. What should I enter on the online form?

I have been a housewife and entered this under employment, but it wants address (I entered my home) and supervisor? What should I enter?

You should choose "Unemployed" so that you can tick "Not Applicable" for the section regarding "Annual Income" and "Work Experience".
