Pakistani citizen living in UAE

Do I need to pay the visa fee in Dubai (UAE) for X1 or X2? I'm a permanent citizen of UAE but my passport is Pakistani

Visa is free for you, but you need to pay AED 210 to the Visa Application Center in Dubai.

And do I need a medical clearance certificate for the same visa? I reside in Sharjah
