Passport failed to be scanned

I went to the embassy in Ghana today, as i was about to be interviewed, i was told i need to do a new passport because the old one cannot be scanned...apparently this has been happening to other applicants. what may be the reason for this, and what is the guarantee that a new passport wont repeat the same scan failure? Im confused
