HELLO, WHAT DOES IT MEAN " Physical Examination Record for Foreigners" WHICH IS REQUIRED FOR X VISA

Also, could it be submitted on my behalf in my absence, by trusted person with my consent and given authority?

Tomasz, " Physical Examination Record for Foreigners" means your physical examination record. Plus, if you apply for residence permit for the first time, you must submit in person.
