About:Chinese Embassies in Asia

I have received jw202 letter but unfortunately my foreigner physical form lost I'm worried

Please tell me how to make process to new physical form??

You can go to the designated hospital in your country to do physical examination and then you can get the physical examination record.

10/11/2018 my old physical form has been lost if I'm going to designated hospital to make new physical form are acceptable?? Tell me please..

I think they would accept new physical form. If you are worried, you are advised to contact the visa office to confirm that beforehand.
