what documentation do I need for a tourist visa to China?

I am planning a trip to China later this month and was wondering what documentation do I need for a visa in addition to a application. I had one previous entry in 2007 as part of a group that entered from Tibet. That Passport is been surrendered and I do not have documents from that visit.
