Hello ..Q1 visa application form

I just want to ask again if how many application form I need to submit in china embassy.. Me and my son will go to china to visit my husband in china..He is a chinese citizen.. What other requirements I need to submit for my son..?does he need an application form also? my son is just 1yrs old.
