US Citizen Sfo 12/20/17 land in Shanghai 12/21/17

departing Shanghai to Pus S.Korea 12/22/17

Where do I apply for the 144hr visa? is there a form?

The 144h twov is NO visa and you can´t apply for it.
Just go to the twov desk at the immigration, show your passport, confirmed ticket to Korea and maybe a hotel reservation and you´re fine.
