Renewal of china passport in Singapore

Hi I am currently in progress with an employee's passport and it shows that his passport is expiring soon. I would like to know where is the application form for passport renewal? I was browsing through the entire page of chinaembassy but couldn't find any application form stating renewal. All I see was visa application form. Please help, urgent!
