Get the application form

Hi,

I am a Vietnamese living in Japan with a working entry permit. I want to go to Hongkong to attend a joint seminar (for business purpose).
I could not find the application form? Could you send me a form.
In addition, should I apply as tourist or business entry permit? Is an invitation letter needed? Do I need to give bank balance?
