Passport renewal. Where do I get the application form?

Hi Gary,
I try to make appointment but it stated no appointment for this month.

Vincent, you can download the application form from the website of the embassy of China in Singapore.
This month will end soon, so the appointment may be full. You can make an appointment for next month.
