As of 6-2017, What Are the Requirements For English Teachers To Apply For Z Visa in Shanghai?

I have livd in China for a number of years now as an ESL Teacher and have found an teaching job in Shanghai. What are the documents I'll need to submit to apply for a Z type / resident permit in Shanghai? And please be specific about which documents need to be authenticated. Thank you!
