I plan on going to China later this year with my girlfriend who was born in china but is now an American citizen. What documents and information would this require?

Jamie, you should prepare the relevant documents: the passport and copy of its data page, a completed application form, and a passport-style photo taken recently, round trip air tickets and hotel booking record. Besides, your girlfriend may also need to provide her previous Chinese passport if she had one before.
