Do i need to fill up 5 different form for my family or only 1 form?

my family and i will be travelling to beijing which i need to apply visa for 5 of us. Do i need to fill up 5 different form for each of us or only 1 form which i can put all my family details in the same form?
Is the form need to fill up first before its being printed or i can just fill in the form by using a ballpen?

You need to fill up 5 different forms for the application.
The answers should be typed when filling out the application form, and then print it out and sign it for your own use.
