I’m afraid you cannot apply for a 5-year work Z type because the work Z type is usually valid for 3 months only. You need to provide those documents for that: your passport and its photocopy, the filled-out application form, a passport-sized color photo, work permit and an invitation letter from a duly authorized unit.0
