You can apply for two separate tourist visas for mainland China and HK.
For mainland China, you need to provide your passport and copy of its data page, the application form, a passport-style photo, round trip air tickets, and hotel reservation.
For HK, you need to provide copy of your passport, a completed application form, a passport-style photo, proof of your financial support, transportation and accommodation arrangement, and your travel plan.0
