how can i attest my documents from china embassy?

i am from dhaka.... i need to attest my certificate from chinese embassy. i am done attesting my documents from ministry of foreign affairs and ministry of education.
1. how will i get and fill up the application form?
2. where will i submit the documents?
3. how much embassy will charge?

You can download the application form the embassy of China.
You should submit the application to the embassy if China for the attestation.
It may cost 2000 Taka, which is for a reference only.
