Form for HK Visa application for PRC Passport Holder

Hi,
Where can I find the application form for the HK/Macao entry permit? I can find the one for HK only, but my previous entry permit that's expiring was a combination HK/Macao entry permit. I have a Chinese Passport and also Canadian Permanent Resident Status. I'm located in Vancouver.
