About:Chinese Consulate in Chicago

Visa Completion

I cannot enter my name or the first three lines of the application but can complete all the rest. Is it OK to write my name and the first three lines by hand writing? Any solution to this problem?

Mark, I’m afraid you cannot write your names and the first three lines by hand writing. It is clearly stated that please type the answer in capital English letters in the space provided. You can download the application form first and then fill it out. I have tried and it worked.
