Applying Q2 visa from Canada

Hi, I have Thai Citizenship. I visited China twice on business 2 years back, but now I have to apply for Q2 from Canada. I want to know if the form will be different from this one on the website of CVASC?

Also, do I need other documents to apply? Thanks so much.

Oor, the form is the same as the one you found.
You need those documents including your passport and copy of its data page, the completed application form affixed with a passport-sized color photo, your valid stay permit to Canada, an invitation letter from the one you visit in PRC as well as the Chinese ID card, or passport and residence permit of the one you will visit.
