About:Chinese Embassy in Zimbabwe

Physical Examination Record Form for student

I got a place at one of the Chinese university and one of the requirements which they say i should bring to the embassy when applying for a visa is a physical examination record form signed by a local doctor from a recognized local hospital. My question is can i use the medical forms from here in Zimbabwe which i get from Kingstons bookshops and is Parirenyatwa Hospital a recognized hospital? or the doctor is supposed to fill the Physical Examination Record For Foreigner?

Hope, as for the recognized hospitals, you should contact with the embassy and consult about it. Usually, a local state-owned hospital is ok.
As for the form, I think it should be the doctor who fill the Physical Examination Record for Foreigner.
