You will need the following documents, including a valid passport, a photocopy of the passport's photo page, a fully completed application form, a rcently-taken color half-length photo, and round-trip tickets and hotel reservations. The HR letter or bank statement is generally not required. If being asked to submit it, I think the bank statement can take place of the HR letter.
