If you apply for the entry permit for mainland PRC, you need your passport, passport copy, application form, passport style photo, round way tickets and hotel bookings. If you apply for the entry permit for HK, you need to submit a photocopy of your passport, an application form, a recently-taken photo, a photocopy of your bank statement, round-trip tickets and hotel orbers and a travel itinerary.
