Is the application form you mentioned the JW 201 or 202 form? If yes, you also need to submit a normal application form V. 2013, your passport, a photocopy of the passport's data page, a color bare-headed photo taken within six months, and a photocopy of the admission letter and JW 201 or 202 form.

its jw202. okay i will take all you listed but whats the normal application form V. 2013? and where do i get it? and do I need to bring bank statement? and how am I going to pay the tuition and accommodation fee which is around $4, 000? can I take it with me to china or do I transfer it by bank?

It's a normal application form applicable to all types of PRC entry permits. You can download it from the website of the embassy. The bank statement is not required in normal cases. As for the payment methods of your tuition and accommodation fee, I think you should check this with your school. If you can transfer it by bank, you had better do so. It's not safe to take so much money when you travel around.

oh! no, I think I have got one big problem. I renewed my passport after I received my jw202 form because only 7 months were left to expire and my new passport has new passport number which is totally different from the one that is written in the jw202. please share your ideas asap I am really shocked.

I don't think it's a big problem. You can show both passports to the consular officer and explain the situation.
