Thank you for the response. Is there any sample for the declaration? Do I also need Certification from the Secretary of State? Your help is greatly appreciated, thank you, Sincerely Steve

Steve, you don't need to make a declaration, but you need to first have both passports notarized by a notary public and then have the documents certified by the Secretary of State. Only after that, you can submit your documents to the consulate for authentication.

Thank you for your answer. How can I get both passports notarized, by making copy of both passports on 1 page, then notarize my signature on the page? Secretary of State, United States or the State where I live? Your help is greatly appreciated. Thank you, Steve

You should notarized the original copies of both passports, not the copy of them and your signature. After the passport have been notarized, you should go to the Secretary of the state where you live to certify the notarized documents.
