what are the other blood tests are required

I have applied for chinese language program in xian jiatong university and i have got admission notice and visa application form from college .

Do i need to fill visa application form also ?

And I have done chest x-ray, HIV, syphilis test and ECG and all tests are normal ,is their any other test required .
And I have the ECG report in Graphical test form . I dont have the written prescription of doctor but doctor has filled-up the health examination form and marked ok on the form with his stamp and seal.
is it ok or do i need the ECG report in written form?
All the tests are done through private hospital and laboratories so for that I have done notary on the photo-copy of the medical documents . Or else do i need to notaries the original docs .?

Sushant, you just need to fill one application form.

It's OK that the doctor fiiled up the health examination form and market ok on the form.

I personally thin you don't need to have the report notarized. If you need to do so, you should notarize the original document rather than the photocopy.

You need to get the physical examination from any govt.hospital not a private .
