Diana, you need to apply for an X1 type for yourself and an S1 for your daughter.Documents respectively needed for both: valid passport, a photocopy of the passport's data page, filled application form attached with a personal pictureSupporting documents for X1: the original and photocopy of your admission letter and JW 201 or 202 form.Supporting documents for S1: the original and photocopy of your daughter's birth certificate and a consent letter from his father.
