Application for Entry for Employment as Imported Workers (ID 1001A) 2. Your recent photograph (affixed on page 1 of the application form ID 1001A) 3. Photocopy of your passport4. Photocopy of proof of academic qualifications and relevant working experience 5. Photocopy of proof of your medical fitness for the job, e.g. medical reportThen ask your boss to submit the application for you.
