why can't i type on the application form beyond the 1st page?

Question

I have tried many times to complete the application on line, but I can never get beyond the first page. When I have completed page 1, I can't type on the following pages for some reason. Can you please help me as I have a medical appointment in China  before the end of the month.I also tried to print the completed page 1, but it would not print.

MJM, I would suggest you first download the application form to your computer, and then fill in all the blanks. After that, you can print the application form.

why can't I type on the application form beyond the 1st page?
