Preeti, you should fill ID1003A form and it can be downloaded from the website of Chinese embassy in Australia. Except for the form, you should provide your passport, a recent passport photo, travel and flight itinerary, accommodation and financial proof.

You are required to submit your application to the consular section of Chinese consulate in Melbourne located at 534 Toorak Road, Toorak, VIC 3142. If the consulate takes care of your application, it takes four working days. If your application was sent to HK, it takes four to six weeks.
