If you come for medical purpose, I think you should apply for an S type. Aside from your passport, passport copy, application form, and recent photo, a dignosis letter or health certificate from a Chinese hospital should be provided. Perhaps a fiancial standing proof will also be required to prove that you can afford to your medical treatment.
