Further question about SAR passport

Hello again,
1. What is the ROP 143? I cannot find anywhere, can you paste a link? Or it is a typo?
2. Does all forms have to be typed? Or handwritten is fine?
3. Can you give me a physical address to the embassy, I am using my GPS.
4. Do you accept credit card or personal check? Do I have to go to make a check via the bank?
5. Some say I need my Hong Kong Birth certificate, I do not have one with me but I do have my HKID card. Is that enough.

It is urgent as I want to make it during the spring break next week April 6 2015. I just want to make sure everything is correct as I need to drive about 5 hours to the embassy. Sorry for the details. Thank you very much.

1. The ROP 143 is a form which can be downloaded from the website of the HK Immigration Department.
2. You had better type all the forms.
3. Add: 3505 International Place, NW, Washington, D.C. 20008
4. The fee for renewing SAR passport is composed by: service fee of the embassy + postage + service fee of HK Immigration Department. You should pay the service fee of the embassy and postage by credit card or cash check. Personal check is not accepted. As for the fee charged by HK Immigration Department, you should include a check in the application documents.
5. If you are elder than 16 years old, the birth certificate is not needed.
