ReplyMalik, you should post all your materials to HK Immigration Department to apply for it. The address is as bellows:
Receipt and Despatch Sub-unit
Hong Kong Immigration Department
2/F, Immigration Tower
7 Gloucester Road
Wan Chai, Hong Kong
The required documents are:
1. finished ID1003A form
2. copies of your passport information page and previous entry permits (if any)
3. financial standings, bank statement, tax receipts or salary slip
