Malik, you should post all your materials to HK Immigration Department to apply for it. The address is as bellows:
Receipt and Despatch Sub-unit
Hong Kong Immigration Department
2/F, Immigration Tower
7 Gloucester Road
Wan Chai, Hong Kong

The required documents are:
1. finished ID1003A form
2. copies of your passport information page and previous entry permits (if any)
3. financial standings, bank statement, tax receipts or salary slip
4. flight ticket and itinerary
