New PRC Passport

My stepson has an expired PRC passport, where are the forms/procedures to obtain Chinese passport. Wife's passport(Chinese) is still valid, does she need anything else for re-entry into Mainland China.

You should go to the consulate or embassy to submit the application for passport renewal. The documents required are: an application form, four 48mmx33mm size color bare-headed photos against white background, the original and photocopy of his passport, the original and photocopy of his US residence permit, and a personal statement.

As for your wife, she doesn’t need the entry permit for mainland.
