Don, you can't get back with your permanent resident card only. You have to apply for a document called declaration of identity for entry purpose from the embassy or consulate. It takes 4 working days to deal with your application. You need to prepare the following documents:
1. registration form for applying the above document.
2. 55mmX45mm color photo
3. copy of your HK passport
4. green card (if you have)
5. filled ID1003A form

Perhaps, the embassy may ask you to apply for PRC Travel Permit. It's ok to use it enter HK.

Can the declaration of identity and other paperwork be sent to the consulate? Or does the applicant have to file in person?
