Entry permit requirements entry through Beijng and exit via Hong Kong

I am visiting China on holiday and want to enter through Beijing but exit through Hong Kong on transit to Australia which visa application form do I need? I hold both Cypriot and British nationality together with passports for both. and have permanent residence in Cyprus. Since my flights are booked using the British passport I would like the entry permit to be processed in this.
