S1 or S2 type and Physical Examination Record required?

Hi,
I have Z-type and i am working in Shenzhen China since November 10, 2014 as Software Engineer in a Software Company.
I want to invite my wife here but i have some questions:
1. Before our Marriage my wife already had NIC and Passport, so in NIC and Passport there is not mentioned that she is my wife, do she need to apply for NIC and Passport again with Husband Name? or Marriage certificate is enough to show to China Embassy.
2. Is it compulsory for her to visit China Embassy in Pakistan to submit documents or her brother can submit her documents and can collect her passport after Visa granted?
3. S1 Type required Medical Examination record but S2 doesn't
Please help me in this regard.
