Medical Check Malaysia

Hello, I am going to work in China soon. My employer request my checks to be done here in Malaysia. Where to get the form and which hospitals recognised by the Chinese government? I heard from an agent that the medical checks must be done in China only. Which one is true? Do I still need to do it in Malaysia?

Supertanker, this form can be found on this website. You can find the "China Visa" link on the right side of TravelChinaGuide's Home Page. By clicking it, you can find the form in the middle of the page.

Since you submit your application to CVASC branches in Malaysia, you may contact them to learn hospitals where you can get the medical examination done.
