Hi, you are first suggested contact the local Chinese embassy to consult whether you need to apply for travel permit or an ordinary visiting type. It is suggested so because you were born in China.
If you need to apply an ordinary one, you will visit your parents or grandparents or other people? In general, you can apply for a Q1. So you need to prepare your valid passport, an application form, a recent taken photo, your kinship certificate, an invitation letter from your family member and the copy of the inviter's ID card and Household register. Then you can apply at the nearby CVASC.
