You can go to Lhasa by air or by train from this city: it takes 25-26h by train and 2h by air.

I'm afraid it is a little hard to get the permit on your own. As foreign travelers have to join a tour to tour in Tibet, you can turn to a travel agency to get the permit. Usually, guests are suggested to submit the application 20 days ahead.

Just tell the travel agency your name (as appears on your passport), nationality, age, sex, occupation passport number as well as the photocopies of your passport and visa and they will help to get a permit. When entering Tibet, you need to show your passport, visa and the permit.
